Scheduling Appointments
To schedule an appointment please call 615-223-9502, or, you may also request an appointment using our convenient on-line appointment request form.
Note: The on-line appointment request form is meant for routine or regular follow-up visits only. Our staff will contact you as soon as possible, during normal business hours, to confirm your appointment time. If your need is urgent, please call our office to schedule an appointment. If you are experiencing a life-threatening emergency, our physicians recommend that you call 9-1-1.
Our office hours are Monday through Friday 7:30 a.m. to 5:00 p.m. Patients are seen by appointment only.
Please bring your insurance card(s) with you to each visit, as well as a list of all medications you are currently taking.
New Patients
If you are a new patient, we ask that you arrive at least 30 minutes in advance to complete necessary paperwork. You may also download our complete registration forms in advance and bring the completed forms with you when you come for your appointment.
Cancellations
If you are unable to keep your appointment, we ask that you call to reschedule at least one day in advance.
